Discovery Guide™

Included are two variations of the Discovery Guide™. Side 1 is the same in both options and is to be used as is. I have included two variations of Side 2. Choose the one you like best.

Also included is an autofill version of the Discovery Guide™. You’ll discover that when you document on side 1, the copy auto-fills to side 2.

Load the versions you like best into your practice management software. Use it on all new and recare patients.

Printing Instructions:

The Discovery Guide™ is designed to be printed as a two-sided document.

Print one per team member

Digital Use Instructions:

1. Complete the form by typing the information in the shaded boxes of each section.

2. When finished, select File > Save As to save the file using the patient’s name.
